Pay Increase Request Form

The Pay Increase Request Form is not for employees to fill out, but rather for managers to complete and submit to the regional office of Spartan Management. This form provides a means for managers to recommend increases based on factors such as achievements, performance and market trends. Any pay increases outside of the annual review process must be approved by the President or Vice President and CEO through the use of this form. By centralizing the pay increase process through the use of this form, Spartan Management is able to ensure consistency and fairness in the salary increase process. It is advisable not to give employees the impression that they will receive a raise prior to completing the formal approval process.
