Job Status Change Form

Per our company handbook, benefits, including but not limited to, holiday pay, floating holidays, vacation pay & 401k are for employees designated as full-time and work 30 hours or more per week. The purpose of this form is to ensure that any employee moving from full-time to part-time or vice versa, acknowledge they their job status impacts the benefits provided. Additionally, the move can also impact an employee’s compensation (hourly and/or commission rate).
