Employee File Management System Authorization Form

This form coincides with our company policy on employee file management. Only authorized personnel may access employee files (personnel, medical & other). It is also expected that authorized access to files, maintain confidentiality and ensure all files are kept secured.

Each employee authorized to access files is to read the policy and sign this form. All forms are to be submitted to the HR department and kept on file in the regional office.
