Employee Benefits Summary

All full-time employees with The Spartan Management Group and companies are provided with a

competitive benefits package. This document is intended to list out all benefits and helpful

information for navigating them.

All new hires will onboard on or before their start date via Paylocity.com. They must remember their

username and login in for all future access for Payroll, election of benefits, requesting time off, etc.

IMPORTANT: All new hires must elect their benefits within 30 days of their start date. Failing to do so

will likely result in not being able to sign up for benefits until open enrollment. Access to elect

benefits via Paylocity typically happens 2-3 business days after their start date.

Open enrollment each year for all employees is in April and takes effect June 1st.

The new hire can follow a link sent to the email they provided when onboarding with Paylocity or by

logging into their account and going to the ‘benefits’ tab (smartphone or computer).

benefits that your location HR liaison cannot answer, you can inquire by contacting our HR department

MEDICAL INSURANCE – Eligible the 1st of the month following 2 months of employment.

Medical insurance is with Cigna. The company provides 100% employee premium paid coverage for an HSA plan with a $3000 personal deductible and a $6000 max out of pocket. Other plans are available at a cost to the employee. Additional coverage for dependents can be elected when enrolling. Once the account is activated, you can create a personal portal at www.cigna.com. An ID card will be mailed and you can also request them online.

DENTAL INSURANCE – Eligible the 1st of the month following 2 months of employment.

Dental insurance is with Mutual of Omaha. The company provides 100% employee premium paid coverage for a Dental Plan that includes annual exams and discounts. Other plans are available at a cost to the employee. Additional coverage for dependents can be elected when enrolling. Once the account is activated, you can create a personal portal at https://www.mutualofomaha.com/employerbased-plans/dental-insurance/employee. They do not mail out an ID card but you can print one after creating a personal portal online.

VISION INSURANCE - Eligible the 1st of the month following 2 months of employment.

Dental insurance is with Mutual of Omaha. The company provides 100% employee premium paid coverage for a Vision Plan that includes annual exams and discounts. Other plans are available at a cost to the employee. Additional coverage for dependents can be elected when enrolling. Once the account is activated, you can create a personal portal at https://www.mutualofomaha.com/employerbased-plans/vision-insurance. Once the account is activated, you can create a personal portal at online. An ID card will be mailed and you can also request them online.

LIFE INSURANCE – Eligible the 1st of the month following 2 months of employment.

All full-time employees will automatically be enrolled in a $15,000 life insurance policy with Mutual of Omaha. When electing benefits, the new hire can increase his/her coverage up to $100,000 without a medical exam. Coverage can also be purchased for qualifying dependents when electing benefits. Following election of benefits in Paylocity, access can be gained via www.mutualofomaha.com.

LONG TERM DISABILITY – Eligible the 1st of the month following 2 months of employment.

All full-time employees will automatically be enrolled in a LTD insurance policy with Mutual of Omaha. When electing benefits, the new hire can increase his/her coverage. Following election of benefits in Paylocity, access can be gained via www.mutualofomaha.com.

SHORT TERM DISABILITY – Not provided but can be purchased when electing benefits.

Employees can elect their own STD when electing benefits. They are encouraged to do so as LTD does not start until someone is unable to work for 6 months. STD coverage will bridge that gap.

401K PLAN – Eligible the 1st of the new quarter following 1 year of employment.

401k & Roth 401k plans are provided through ABP Retirement. Once eligible, employees can create a portal at: www.benefitwebaccess.com/apb. The company will match up to 4% based on the plan criteria.

PAID HOLIDAYS – Eligible for all full-time employees after 6 months of employment.

Company Holidays are New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day and Christmas Day. We maintain normal business on all holidays except for Thanksgiving Day and Christmas Day (office is closed but emergency coverage is provided for our customers). The GM will determine all holiday coverage. To qualify you must work your scheduled day before and after the holiday and it cannot be combined with other time off. You can refer to our Company Handbook and Holiday Policy for more information.

FLOATING HOLIDAYS – Eligible for all full-time employees after 1 year of employment.

All full-time employees receive 2 floating holidays which can be used after vacation pay has been exhausted. Due to sick pay stipulations Colorado, 1 floating holiday is allotted and can be used only after any accrued vacation time has been exhausted.

VACATION/SICK PAY – *Varies by State.

States other than WA and CO - After 1 year of full-time employment, an employee qualifies for 40 hours of vacation pay; after 4 years of employment, 80 hours; and after 9 years of employment, 120 hours. A max of 40 hours can be rolled over into a consecutive year. Unused vacation will be paid out upon separation of employment on the final paycheck.

*For Washington employees (years 0-5) - 1 hour of sick pay is accrued per 40 hours worked, which can be used as vacation pay. A maximum of 40 hours can be rolled over but no more than 80 hours can be accrued. Sick pay is not paid out, should there be a separation of employment. After 4 years of employment, the employee qualifies for 40 hours of vacation pay; and after 9 years of employment, 80 hours. Unused vacation will be paid out upon separation of employment on the final paycheck.

*For Colorado employees (years 0-5), 1 hour of sick pay is accrued per 30 hours worked which can be used as vacation pay. A maximum of 48 hours can be rolled over but no more than 96 hours can be accrued. Sick pay is not paid out should there be a separation of employment. After 4 years of employment, the employee qualifies for 40 hours of vacation pay; and after 9 years of employment, 80 hours. Unused vacation will be paid out upon separation of employment on the final paycheck.
